Advance Registration Methods
Enrollment for all courses is limited. Registrations are accepted and assignments are made in the order they are received. Advance Registration will close on October 25, 2011. There are 3 easy ways to register:

On-line - http://www.aana.org and click on 2011 Fall Course. Click on the registration link and complete the registration form along with your credit card information. PLEASE NOTE – when you register on-line you will receive your confirmation by MAIL. You will NOT receive an automatic confirmation via our website.

Fees
The registration fee includes all continental breakfasts, coffee breaks, reception, registration materials, lecture sessions and laboratory sessions.

Knot Tying Laboratory
A Knot Tying Laboratory will be available for the Shoulder Lab participants. Instructors will be available on:

Lunch in the Exhibits
A box lunch will be served in the exhibit hall on Thursday and Friday of the course. Please indicate on the registration form if you would like to request a lunch for one or both days.

Laboratory Requirements & Instruments
Each laboratory station will have two registrants per cadaveric specimen. Universal precautions will be required in the laboratory. Gowns, gloves, masks and goggles will be provided. SCRUBS ARE NOT PROVIDED IN THE LABORATORY.

***PLEASE NOTE*** The shoulder model lab and the knee model lab will be integrated in to the cadaver lab sessions to enhance and supplement the lab experience.

Reception
A reception will be held on Thursday, November 17 at 7:00 pm in the Exhibit Hall at the JW Marriott Desert Springs. Admission to the reception is included in the registration fee.

Refunds
Refunds will be subject to a non-refundable $75.00 processing fee. Requests for refunds must be received in writing at the Association office by October 25, 2011. No refunds will be issued after October 25, 2010.

Rental Cars
Hertz has been appointed the official rental company for this course and is offering special discounted rates. Reservations may be made by calling the Hertz Meeting Sales Desk at 1-800-654-2440 and identifying the group name and meeting number: CV#024M0019.

Airline Travel
The Arthroscopy Association of North America has appointed Gant as the official air travel coordinator for the Fall Course. Gant provides attendees with personalized, unbiased airline reservations and ticketing at the lowest available fare with one toll-free call. To receive the exclusive AANA air fares, call Gant at 1-800-621-1083. If you are outside of the U.S. and Canada, please call 630-227-3800. Gant will charge a minimal transaction fee of $22.00 per airline ticket issued.

Waiver
Routine safety precautions should be taken by all participants as during normal surgical procedures. A signed waiver of liability will be required from each laboratory participant. Participants will be sent a waiver which must be signed on-site at the time of registration.

Golf Tournament
AANA/BREG Golf Tournament
Saturday, November 19 at 12:30 pm
A shot gun golf tournament has been planned for course participants at the JW Marriott Desert Springs. A $200.00 greens fee should be returned with your registration form. This includes one round of golf, range balls, cart, prizes, food, contests and more. The registration fee for each player will be donated to support the AANA Education Fund. Our sincere thanks to BREG, Inc. for organizing this event. Registration is limited to Course Registrants only (Members and Non-members). Advanced registration will be required with the participant’s official handicap or average score. Please note: No refunds after October 25, 2011. After this date the tournament fee will be considered a donation. Maximum number of participants: 72

Walk for Dr. Leslie Sgaglione Academic Scholarship Fund
Friday, November 18, 2011 at 9:00 am
Bring your walking shoes and your family to help us celebrate the life of Dr. Leslie Sgaglione. Join us for the AANA Pals walk dedicated to her memory. All donations can be made to the Dr. Leslie Sgaglione Academic Scholarship Fund to benefit the students of the North Shore School District in Sea Cliff, NY. Donations will be collected at the time of the walk.
