The application for the Active, Associate, and International categories is a two step process.  First, the applicant needs to obtain two sponsors.  Once both of the sponsor forms arrive in the office, then the actual application is mailed to the applicant. There is one exception, Associates may use their Fellowship Director; then one sponsor will qualify. Check to see if your sponsor forms have arrived in the office by entering your last name below. If both of your sponsor forms have arrived in the office, then you will be allowed to download the actual membership application.

The Resident/Fellow category is a one step process which requires a letter of recommendation from a Residency Director or Fellowship Director along with the application. The Resident application can be downloaded from our Membership Categories page.

Deadlines

The deadline for Active applications is October 1 st of every year. Candidates will be notified after the Annual Meeting in the Spring of their membership status.

Associate, International, and Resident applications are processed as they come into the office throughout the year.
