FEES

The registration fee includes all continental breakfasts, coffee breaks, reception, registration materials, lecture sessions.

Continental breakfast and coffee breaks will be held in the Exhibit Hall located in the West Foyer.

DISCLOSURE STATEMENT

In accordance with the Accreditation Council for Continuing Medical Education standards for commercial support, all CME providers are required to disclose to the activity audience the relevant financial relationships of the planners, teachers, and authors involved in the development of CME content.  An individual has a relevant financial relationship if he or she has a financial relationship in any amount occurring in the last 12 months with a commercial interest whose products or services are discussed in the CME activity content over which the individual has control.

To compliment the education session, a variety of exhibits will be on display throughout the Fall Course.


LUNCH IN THE FOCUS DEMONSTRATIONS

A box lunch will be served for Focus Demonstration registrants only outside of the Focus Demonstration ballrooms on Thursday and Friday.  Please indicate when you register what Focus Demonstration you plan to attend.


MINI FELLOWSHIP LABORATORY REQUIREMENTS & INSTRUMENTS

Universal precautions will be required in the laboratory.  Gowns, gloves, masks and goggles will be provided.
*Scrubs are NOT provided in the laboratory - Please bring your own


PHOTOGRAPHY

Registration and attendance at or participation in AANA Fall Course and other activities constitutes an agreement by the registrant to AANA’s use and distribution (both now and in the future) of the registrant or attendee’s image.


WELCOME RECEPTION

AANA welcomes all attendees and their guests to the reception on Thursday, November 1th at 7:00 PM in the Exhibit Hall located in the West Foyer.  The Welcome Reception is a wonderful way to reunite with old friends and network with colleagues.  Admission to the reception is included in the registration fee.


VIRTUAL GOLF

Enjoy the ultimate in golf simulation presented by AANA’s Education Foundation. This will be our first-ever virtual reality golf simulator from Amusitronix featuring swing analysis, longest drive contest, closest to the pin, and regular play. Keep your swing up to par in this final round of AANA’s Fall Course.

REFUNDS/CANCELLATIONS

Refunds will be subject to a non-refundable $150.00 processing fee.  Requests for refunds must be received in writing at the Association office by October 20th, 2017.
*No refunds will be issued AFTER October 20th, 2017


WAIVER

Routine safety precautions should be taken by all participants as during normal surgical procedures.  A signed waiver of liability will be required from each laboratory participant.
